Increase in AIDS Cases by Sex

What is Cervical Cancer?

Cervical cancer is a preventable disease, through cervical screening and the human papilloma virus (HPV) vaccination programme.

Despite this, 20 per cent of women didn’t take up their invitation for cervical smear tests last year. In addition, only 50 per cent of the girls offered the HPV vaccine have chosen to have this potentially life-saving jab.

We look at the issues surrounding the vaccine.

Cervical cancer and HPV

Cervical cancer is a disease that causes distress to thousands of women and their families. Every year around a 1,000 lives are lost, with many other women faced with potentially traumatic treatment and sometimes surgery.

Around 99.7 per cent of cervical cancer cases are caused by the human papilloma virus (HPV), which is transmitted during sexual activity.

But with a new vaccine on the market, the future should present a far better outlook – with cancer charities hoping the rates of cervical cancer can be significantly reduced.

So what has all the controversy been about?

What is HPV?

HPV is a common virus that affects the skin and moist membranes in the body and is passed on through contact with an affected person.

There are around 100 different strains of HPV, with around 40 affecting the genital area. Other strains of HPV affect other areas of the body, such as the mouth and throat.

Seventeen strains of HPV are related to cervical cancer. These are among the high-risk strains of HPV. Low-risk strains – including those affecting the genital areas – can lead to verrucas, warts and genital warts.

‘Around 75 per cent of women will catch the HPV virus at some point in their lives. For the majority of these women, their immune system will get rid of it,’ says Robert Music, Director of Jo’s Trust, a charity dedicated to women affected by pre-cancer and cancer of the cervix.

‘For a small percentage of women, the virus may stay in their bodies much longer, possibly leading to abnormalities that can develop into cervical cancer,’ he says.

What is the HPV vaccine?

The cervical cancer vaccine has been developed to help prevent catching high-risk strains of HPV.

There are currently two types of vaccine on the market:

  • Cervarix, which is offered in the UK immunisation programme
  • Gardasil, which is offered in the US immunisation programme.

Both protect against the HPV strains 16 and 18 that cause most cases (70 per cent) of cervical cancer. The Gardasil vaccine also protects against strains that lead to genital warts.

Since September 2008, an HPV immunisation programme has been offered to 12- to 13-year-old and 17- to 18-year-old girls. This consists of a course of three vaccinations being administered over a six-month period.

A catch-up programme was also announced at this time with 13- to 18-year-old girls being offered the vaccine over the following two academic years.

What’s the controversy?

Since the introduction of the vaccine, several newspaper headlines have questioned the link between the HPV vaccine and symptoms reported by some girls, after having the jab.

Some of these symptoms have been reported as severe and debilitating.

In September 2009, a 14-year-old girl died shortly after being given the cervical cancer vaccine, leading to the Medicines and Healthcare Products Regulatory Agency (MHRA) recalling the batch of vaccinations from which she had been given her injection.

But it was later discovered that the young girl had underlying health problems and died of a tumour in her chest.

According to an MHRA report, there have been 3,027 reports of adverse side-effects and 6,940 suspected reactions. But these include many of the lesser symptoms, such as pain at the injection site.

The campaigning group Jabs say they have spoken to parents in around 18 cases, who are worried about the illnesses their daughters have developed.

‘If you’re vaccinating a large number of people, by coincidence other things will happen at the same time,’ says Dr Anne Szarewski, clinical consultant for Cancer Research UK, and a leading expert in cervical cancer.

‘This is especially the case when you’re looking at such a large figure – around 1.4 million have been vaccinated. So, by the time you have vaccinated them, you’re bound to have all sorts of things that happen at the same time,’ she says.

Possible side-effects from the vaccine range from:

  • pain and redness at the injection site
  • aching muscles
  • headache
  • gastrointestinal symptoms
  • fever
  • uncommonly, dizziness and infection of the nose and throat.

Elsewhere, a small number of religious groups have questioned whether having the cervical cancer vaccine will lead to promiscuity. In the US, there’s only been a 25 per cent uptake of the vaccination. However, contray to this, studies have shown that the cervical cancer vaccination makes girls think twice about having sex.

‘The vaccine has been thoroughly researched, and Jo’s Trust believes it’s an important programme that could save up to 400 lives a year,’ says Robert Music.

‘Over a number of years that becomes thousands,’ he says.

Who else should take the vaccine?

Since HPV is caught through sexual activity, the vaccination programme hopes to catch the majority of girls before they become sexually active.

But given the nature of the HPV, it makes sense to have the vaccine at any age. Currently the vaccine is available privately and from Boots the chemist.

‘It’s never too late to get the vaccine,’ says Dr Anne Szarewski. ‘You can catch HPV at anytime in your life, especially with the pattern of changes that are now taking place in people’s lives, such as people meeting new partners in their 40s and 50s. So it’s never too late to have the vaccine.

‘The other thing is, even if you catch it and develop a natural immunity, it’s likely to be so poor that you get affected again by the same strain.

‘And if you have had a strain of HPV previously, and your body’s developed some antibodies, the vaccine is thought to act as a booster. The only time it won’t help is if you have an ongoing HPV infection,’ she says.

There’s also an ongoing debate about whether the cervical cancer vaccine should be given to boys.

Cervical screening

The important thing to remember is: even if you have had the HPV vaccine, it’s vital to still attend cervical screening.

The cervical cancer vaccine only protects against two strains of HPV (16 and 18) – the most common causes of cervical cancer. But the vaccine doesn’t protect against all cervical cancers. Worryingly, rates of attendance for cervical screening are dipping.

‘Around 1 in 3 women aged 25 to 34 are not taking up their invitation. It’s important to get the message across that women should go for a smear test because it’s a programmer  that can save lives,’ says Robert Music.

‘Even if you have been vaccinated, you should attend cervical screening. Some young women may think because they have been vaccinated against HPV, they don’t need to have a smear test.

‘But it’s important that they do because the vaccine only protects against 70 per cent of HPV strains,’ he says.

What else can you do?

As well as regular smear tests, a healthy immune system can help combat HPV, as can giving up smoking.

Smoking has been shown to double the risk of HPV.

‘Wearing a condom may also help, but that’s not necessarily going to be 100 per cent effective,’ says Robert Music.

Menopause is the cessation of menstruation. As menopause nears, the menstrual cycle can become very sporadic. Some women will skip a period or not have one for several months at a time. A diagnosis of menopause is made when a women has not has a menstrual period for 12 consecutive months.

The North American Menopause Society estimates nearly 40 million American women had reached menopause by 2000. In the U.S., the average age at menopause is 51, with a typical range of between 40 and 58.

As women reach menopause, production of the hormones, estrogen and progesterone, decline. Lower and erratic levels of hormones can lead to a variety of problems. One of the most common symptoms associated with menopause is hot flashes (the sudden sensation of extreme body warmth, sometimes referred to as hot flushes). The brain’s thermostat mistakenly believes the body to be too warm and attempts to cool down. Blood vessels near the surface of the skin widen to allow more blood to reach the cooler surface, causing flushing in the face and neck. Some women also experience profuse sweating in an attempt to cool the body. A hot flash generally lasts from one to five minutes. As the symptoms reside, some women may feel chilled, cold, and clammy.

More than two-thirds of women experience hot flashes in the time leading up to menopause. The number and degree of hot flashes vary. Some women may only have an occasional hot flash, while others may have them several times a day. Hot flashes that occur at night (called night sweats) can interfere with sleep.

Acupuncture for Hot Flashes

Traditionally, doctors have prescribed hormone replacement therapy (HRT) to reduce some of the symptoms of menopause. HRT has also been used to reduce the increased risk of osteoporosis, elevated cholesterol, and heart disease in women after menopause. But not all women can tolerate HRT. Some women have also become concerned about the safety of HRT after researchers recently halted a major trial of hormone replacement (due to an excessive number of cases of breast cancer, heart disease, and stroke).

One alternative form of medicine is acupuncture. According to traditional Chinese medicine, two forces – ying and yang – are manifested in the form of a life energy known as Qi. This Qi flows through the body along specific pathways known as channels or meridians. Any disruption in the flow of Qi can cause symptoms or illness. Placement of fine needles along the appropriate pathways lead to a balance of Chi and a restoration of health.

A study published last summer in the journal, Obstetrics and Gynecology, found more than 10 percent of women were using acupuncture for relief of menopause symptoms. Research suggests the treatment may significantly improve hot flashes and some of the other physical symptoms of menopause. Some acupuncturists recommend herbal medications, such as dong quai, in addition to the acupuncture treatments.

The potential benefits of grape juice         

Grape juice fights arterial disease – that was the headline of the Reuters dispatch referring to an article in the journal Circulation. Grapes, like nuts, seeds, some vegetables and fruits, contain flavonoids – interesting substances that are also found in red wine and act as antioxidants. The investigators studied twelve men and three women, average age 62 years, who had known coronary heart disease. They were asked to drink Welch’s 100% Concord Grapes for fourteen days – about 21 ounces or two-thirds of a quart a day. Studies were then made of the diameter of an artery in the arm and the ability of low-density lipoprotein cholesterol (LDL, so called bad cholesterol) to be oxidized, a step thought to be important for LDL to get into blood vessel walls and cause arteriosclerosis.

The results showed that, after the grape juice diet, the blood vessels showed a greater ability to dilate and the LDL was more resistant to oxidation, both reactions likely to reduce the risk of coronary heart disease and heart attacks.

Commentary: This is a good preliminary study. But, that is all it is; whether findings in the arm can really be directly related to blood vessels of the heart is not clear.

The authors of this study point out that the beneficial effects of red wine may not be related to the alcohol, but rather the flavonoids in the grapes used to make the red wine. The flavonoids are a complex group of chemicals with different, but often overlapping, functions. Their names are not exactly household words – querecetin (a very active antioxidant), myricetin, catechin (a powerful inhibitor of LDL oxidation), kaempferol, tannic acid. Of course, it is also true that the reported beneficial effects found in this study may be due to substances in grapes other than flavonoids.

Alcohol, in particular red wines, appears to be beneficial in reducing heart attack risk. That benefit has been ascribed primarily to raising blood levels of protective high density lipoprotein cholesterol (HDL-good cholesterol) and diminished clotting tendencies of the blood that can be the precipitating event for a heart attack.

In this study, the grape juice did not increase the HDL levels. The interference with LDL oxidation does appear to be an important factor in reducing the likelihood of developing arteriosclerosis, but this has not yet been definitely proven.

Variated of watermelon

There are more than 1200 varieties of watermelon, ranging in weight from less than a pound to more than two hundred pounds, with flesh that is red, orange, yellow or white.

  • The Carolina Cross produced the current world record watermelon weighing 262 pounds (119 kg). It has green skin, red flesh and commonly produces fruit between 65 and 150 pounds (29 and 68 kg). It takes about 90 days from planting to harvest.
  • The Yellow Crimson has a yellow colored flesh. It has been described as sweeter and more honey-flavored than the more popular red flesh watermelon.
  • The Orangeglo has a very sweet orange pulp, and is a large oblong fruit weighing 9–14 kg (20–30 pounds). It has a light green rind with jagged dark green stripes. It takes about 90–100 days from planting to harvest
  • The Moon and Stars variety was created in 1926. The rind is purple/black and has many small yellow circles (stars) and one or two large yellow circles (moon). The melon weighs 9–23 kg (20–50 pounds). The flesh is pink or red and has brown seeds. The foliage is also spotted. The time from planting to harvest is about 90 days.
  • The Cream of Saskatchewan consists of small round fruits around 25 cm (10 inches) in diameter. It has a quite thin, light green with dark green striped rind, with sweet white flesh and black seeds. It can grow well in cool climates. It was originally brought to Saskatchewan, Canada, by Russian immigrants. The melon takes 80–85 days from planting to harvest.
  • The Melitopolski has small round fruits roughly 28–30 cm (11–12 inches) in diameter. It is an early ripening variety that originated from the Volga River region of Russia, an area known for cultivation of watermelons. The Melitopolski watermelons are seen piled high by vendors in Moscow in summer. This variety takes around 95 days from planting to harvest.
  • The Densuke Watermelon has round fruit up to 25 lb (11 kg). The rind is black with no stripes or spots. It is grown only on the island of Hokkaido, Japan, where up to 10000 watermelons are produced every year. In June 2008, one of the first harvested watermelons was sold at an auction for 650 000 yen (6300 USD), making the most expensive watermelon ever sold. The average selling price is generally around 25 000 yen (250 USD)
